Services are provided to seniors who have been assessed and have a formal plan in place with a professional social worker. Services may include counseling, information and referral on benefits and community resources, application assistance, financial management, long term care planning and regular follow-up contact.
Case Assistance
Services are provided to seniors who need professional social work assistance on a short-term basis. These services include answering questions regarding quality of life and entitlement services, referrals to community resources and assistance in completing entitlement forms and applications.
Subsidized Services
Transportation to medical appointments, housekeeping, personal emergency response systems (PERS) are available to seniors at an affordable rate.
Healthcare Management/Assistance
Registered nurses provide blood pressures screenings, evaluate individuals for services and are available to answer health-related questions. Health screenings and wellness lectures are offered to seniors throughout the year.
Recreation and Group Services
A broad array of weekly and monthly health promotional, educational and recreational programs are available to seniors in the community. These may include movie day, exercise classes, coffee house lectures, discussion groups, group trips, parties, and intergenerational programs.
Group Transportation
A mini-van, funded in part by the Queens Borough President, provides weekly transportation to supermarkets, banks, shopping malls and cultural/recreational points of interest to ambulatory and wheelchair bound seniors.
Volunteers
A variety of volunteer opportunities for both seniors and younger adults in the community are available, which may include but are not limited to the following: shopping, telephone reassurance, office/clerical duties, advisory council membership, recreational instructor, trip planning and fundraising.
